I stand on my feet all day at work and have pain in my lower back which shoot down both legs, sometimes I can ease it by stretching my back and other times it just won’t go away.

The key points about most back pain and sciatica are:

Do not worry – it will usually get better on its own, within a few days, or possibly a few weeks.
Stay active, and if at all possible, stay at work.
The pain may force you to rest, but this is a result of the pain, and not a good treatment for back pain and sciatica. If you have to take to your bed, limit it, if possible, to a week or two at the most.
Use simple pain killers, such as paracetamol or ibuprofen if necessary.
Avoid activities likely to put unnecessary strain on your back.
See your doctor if you think you are in one of the red flag categories, or are worried about how you are doing.
If it seems to be lasting more than a few days, consider seeing a physiotherapist, chiropractor or osteopath. You may wish to consult your doctor prior to this.
If you do have to be off work, keep this to a minimum, and do not expect to be pain free before you go back.
If the pain is severe and very disabling, and you cannot control it with simple pain killers and/or anti-inflammatory medication, your doctor may suggest you have an epidural injection, which is an injection into the spine, which soothes the nerves that are causing the pain.
In a minority of cases, the sciatica does not settle, or complications arise. This may lead your doctor to recommend surgery, which will involve trimming back bone or disc material which is pressing on the nerve or nerves. This is not undertaken lightly, and will be guided by the results of a CT or MRI scan.

i have been suffering low back pain since June 2007. i am trying to treat it without meds. so i get a massage twice a month, and get "active release technique at my chiropractor" it seems to be working. this might help.

I have ankylosing spondylitis and was taking enbril but due to insurance issues can no longer afford it what are some exercises I can do to keep on top of my lower back inflammation?

Here are two good back exercises that should help you get some relief. The second one you can do all the way to the end of your tailbone if you want:
Back:
Place your left hand on your left knee. Place your right hand over your left shoulder and with your fingertips find the muscle next to your spine. Press on it and hold. Relax, take a deep breath and exhale and don’t tense up any part of your body. After about 30 seconds there should be a release happening and when it does slowly lower yourself forward onto your right leg. If you can lean over the outside edge of your leg it will be better for your release. Continue holding for a total of one minute. Then release but rest your body there for one minute longer. Then reverse and do the right side.
For lower back, on the buttocks:
Put both hands behind your back and place them on the muscles alongside your spine and press on them and hold. Now relax, take a deep breath and exhale and don’t tense up any part of your body. When the release starts to happen, slowly lower yourself forward as far as you can go. Then release the pressure but hold your body there for one minute longer.

Use a heating pad for 20 minutes, ice for 20 minute and back to heat for 20 minutes.

My friend has pretty severe ongoing upper back pain from a past accident. He tells me the doctors have done all they can. Would massage help? Are there any other types of treatment available?

Chiropractic spinal adjustment is the standard of care for these sorts of problems. Back pain, Neck pain, Hip pain, etc. are all commonly treated (effectively) by chiropractic treatments alone.

EDIT: in reply to the previous person’s answer…chiropractors are doctors who adjust the spine to remove misalignment. They might work on muscles via muscle adjustment, but it is a wrong misconception to call it massage. Chiropractors, in general, do not masage. The treatment that works is chiropractic adjustment.
